Phoenix House Mid-Atlantic Welcomes New Role of Discharge Coordinator

Erika Bruce- Discharge Coordinator

Phoenix House Mid-Atlantic’s Assessment Team has introduced the new position of Discharge Coordinator. Filling this role is Erika Bruce, who started her career in the 1990s working with at-risk clients across the lifespan. She continued her work in the field as Director of Social Services for a skilled nursing facility, coordinating discharges, including wrap around planning with a full continuum of step-down services.

Erika received her Bachelor’s Degree in Social Work with a minor in Psychology from Concord University. She then completed Master’s level courses in Clinical Psychology from Marshall University. She worked as a community coordinator and case manager in Monroe County, WV, where she supervised adult and peer mentors, taught interactive research-based curriculum within middle and high school classrooms, and co-wrote and facilitated a grant to conduct research on substance abuse and violence prevention in schools. As a medical case manager, Erika acted as a liaison with medical professionals, patients, caregivers, and community services to ensure that patient needs were met in accordance with treatment plans. Erika coordinated care plans including setting up therapies and medical appointments.  As a foster care recruiter, Erika gained valuable experience in developing and maintaining relationships with community resources and networks.

In her role as Discharge Coordinator, Erika will focus her efforts on working with clients and staff to help connect the client with the needed aftercare resources. This process starts the day the client is admitted and continues through to discharge. Erika will develop and implement a uniform discharge planning process including contact and interaction with the client and family to help ensure understanding of and engagement in the discharge plan.

On a personal level, Erika loves staying active. She became a newlywed last September and her husband introduced her to the game of golf. Erika is a proud parent of Ariel, Erin, and Logan. Erika was previously a ski instructor and also loves any activity near the water. Phoenix House Mid-Atlantic is excited to welcome Erika and her talents to the team!
