Family Wraparound Services
Santa Ana, California
For teens up to age 18 who have a drug or alcohol use history, we create care plans with their support network to address behaviors and mental health, and to identify goals.
Our Family Wraparound teams work collaboratively with family, school, and community support systems to identify underlying drivers that are leading to disruption in the home environment, substance abuse, and mental health issues for teens referred to us in the Los Angeles and Orange County areas.
Youth Partners help patients recognize substance use disorder and addiction issues by identifying triggers and the risks involved with continued use. Our teams work with teens to establish needs and goals to increase academic and social-emotional success.
Parent Partners provide day to day support to caregivers to secure basic needs, address housing or employment issues, and offer guidance in enriching family relationships.
Lead by a Care Coordinator, the whole team works to implement creative interventions to help families function independently, across settings, and to the best of their ability.
